Session II : “The 4 Ms: A Framework for an Age Friendly Health Care System”

 The majority of older people want to age in place, which is possible with the right support and planning. This multi-part roundtable series will bring together experts and thought leaders to help us understand some of the vital components needed to age well in the community.

 Session II will offer information about the 4M framework for a patient-centered health system designed to meet the needs of older adults. The 4 Ms of good health care are: 1) what Matters to you, 2) Mobility, 3) Mentation, cognitive and mental health wellbeing, and 4) Medications to support your health goals. Presenters will share the work of the John A. Hartford Foundation to promote initiatives that support age-friendly care, including their emergency room pilot program designed to meet needs of older patients in the ER. 

 The January 20th Roundtable will feature Rani E. Snyder, MPA, Vice President for Program, The John A. Hartford Foundation, Jane Carmody, DNP, MBA, RN, FAAN, Senior Program Officer at The John A. Hartford Foundation, and Maria Torroella Carney, MD, Chief of Division, Geriatric and Palliative Care Medicine, Long Island Jewish Medical Center.

 You will have the option of joining the event online through Zoom, a webinar hosting service. 

You will also have the option to view the event online through Facebook. Please note that you do not need a Facebook account or profile to view the event through Facebook.
